BEFORE & AFTERCare Program Registration 2022-2023
Parkhurst Academy is pleased to offer an Extended Care Program for the 2022-2023 school year. Before care is offered for all students from 6:45 a.m. until the start of school. Aftercare is available from the end of school until 6:00 p.m.
In order to utilize the Extended Care Program, each family must have a signed registration form prior to a student attending our Extended Care Program. A fee of $5.00 per hour, per student, will be assessed for use of the extended day services. This charge will be assessed for anytime spent in extended care during that hour. A charge of $10 will be assessed for any student picked up after 6:00pm. Every minute of care provided after 6:05pm, will result in a $1 per minute charge.
ALL PAYMENTS FOR THE COMING SCHOOL YEAR WILL BE COMPLETED THROUGH FACTS ACCOUNTS
By subsmitting this form, I agree to enroll the child listed above in PA Extended Care Program. I have indicated the days my child will attend. I agree to pay fees on the due date in FACTS. I understand that failure to pay charges by the due date will result in a $10 late fee, and that in the event that my bill is past due over 30 days my child will be removed from the Extended Care Program.
